People are often scared of things they don't know. In Health and Safety this is often seen in the avoidance of employers to create the programs needed detailing how they will protect their work environment. Plus it's extra work for somebody who generally doesn't know what to do.
What to do? Don't make your own! Don't try to start from scratch.
Whatever you are doing someone has already considered those issues. Go online, type in what type of safety plan you are looking for. Copy someone elses as a starting point and then customize it to fit your site. The government, colleges, big companies, all have spent lots of money making many safety programs that are posted online. OSHA (state and federal) have posted templates for the most used programs that are there for you to use.
I like getting several so I can see more views on a given issue. Then I copy and paste, rewrite, and abbreviate. That last bit is important. People write too much. If you want someone to read something keep it short.
Now you can make the best programs in the world and update every time a change is needed but if OSHA shows up on site and asks an employee or a supervisor about one and they seem to have never heard of it, that inspection is not going to go well. Whatever you expect your employees to know how to handle you have to train them on.
If you want these programs to work, you need to train the appropriate people so they can react correctly when it's needed. They may forget, they may panic, but if you've never told them what to do, you can be pretty sure it's not going to go to plan.
I've included some sample plans below. Some are definitely the long worded versions and could be shortened but it's better that you see things that you might need than not to include them. As I said before you need to read them, customize them, and then distribute it appropriately.
